PA Department of Health Immunization Requirement –tells which immunizations/exams are needed for which grades.
BCS Medical and Dental Forms–You may use these forms or another preferred form from your practitioner.
BCS Educational Trip Form–Use this form if you are taking a vacation during the school year.
Clearances — BCS encourages parents and grandparents to volunteer in our school. We believe that volunteers make a valuable contribution to our students, faculty, and staff. In order for you to support our school, proper clearances must be obtained. Volunteers with direct supervision over children that might read to children, fill in as a field trip chaperone, help out with Run Club or coach will be required to file all the clearances below. Volunteers will be required to re-certify every five years.
**NOTE – If you have lived in PA for ten years you can sign an Exception Waiver instead of the FBI fingerprinting. A waiver may be given upon request.
